This is to inform you that I have membership of your gym and would like to cancel the same. My membership number is 1234.
I am working for Pharmaceutical firm and have recently got transferred to US permanently. Therefore I will not be able to continue my membership in your gym. I made my full payment on 25th April, my cheque number is 12324.
I request you to please approve my cancellation and refund my membership amount. I know there are cancellation charges which I am fine to pay the charges. Please let me know when I get the balance amount.
