Subject: Cancellation of the Gym Membership
Dear Mr. Johnson,
I am writing this letter to inform you that I would like to cancel my gym membership of Talwalker, I am have recently joined your gym and I must say your instructors are doing a great job and have helped me maintain a fit body.
But recently due to heavy weight lifting I got a serve back strain, and doctor has advised me for bed rest and asked to not work out in gym due to my health issues.
If required I can give the medical certificate for your reference. I request you to cancel my membership and give me the refund. I know there are cancellation charges that’s need to be paid therefore you deduct from advance payment and refund me the rest balance amount.
