Dear Sir/Madam,
I have been admitted to your hospital for several months. During this period I have undergone various tests and checkups. Event after all these things I have yet not noticed any improvement in my health. I am in great dilemma and scared about my health and need some answers.
I would like to make appeal to please make me aware about the truth. I would like to know exactly what happened to me because of which I have been admitted to this hospital for so long. I have yet not received any reports of the different tests and checkups that I have undergone.
I hope that you will make response to my appeal as early as possible and help me to get out of various dilemmas.
