Re: Application for Maternity Leave
Dear Mrs Jones,
This is to inform you , that I am 8 months pregnant and my first child is due in mid may 2010.I would request you to grant me maternity leave from the 1st of May 2010. All the required forms have been duly filled and attached for your perusal.
If you check my last one year leave records, then you will find that I have not availed of any leave. I request you to add my pending unavailed leave of last years to my maternity leave. My tentative date of joining would be 1st of November 2010.Kindly confirm, if I would be eligible for my salary during my leave period. Also let me know if anything else needs to be done from my end.
