Dear Mr. Matthew,
I am writing this letter to request you a leave of absence for about two weeks starting from 6th Aug 2010 for the reason given in this letter. My mother has been diagnosed as having a cyst and needs to undergo surgery for the same. She needs some to accompany her to the hospital for the surgery and also needs someone to be with her during recovery. The entire process, according to the doctor needs two weeks.
As my mother and I are the only ones in the family, and it is obvious that this responsibility will fall on my shoulders. Hence I request you to grant me a leave for a period of two weeks starting from 6th Aug. I shall resume work on 19th Aug 2010, but if there is any problem, I shall give you prior notice of delay.
