Dear Mr. Smith,

I would like to cancel my credit card that I am holding with effect from 5th June, my credit card number is 123456 and in name of Paulson.

Due to recession I am unemployed from last 4 months and there is enough outstanding on me to pay. Therefore I request you to cancel my credit card. I have settled all my account with your bank and there is not outstanding left from my end.

I have never faced any problem while using the card from your official, I appreciate for your service. Please accept this letter as a formal notice for cancellation of my credit card and let me know if any other formalities need to be completed for the same.
